How to Complete Your LiveText Forms

How to complete your LiveText forms

Internship Supervisors, Site Supervisors, Mentors, and Cooperating Teachers: To complete your LiveText forms:

  1. Open the email that contains the link(s) to your LiveText forms
  2. Open or print the attached rubric(s) for your reference when completing the form(s).  Some forms do not require rubrics.  You will not have a rubric attached to your email if your form(s) are of this type.
  3. Click OR copy and paste a link into your Internet browser
  4. Type or click the drop-down boxes to enter the information requested and/or your comments
  5. Click "Submit form"
  6. Repeat the above for each form link in your email, as appropriate.

Citadel Faculty: To complete your LiveText forms:

  1. Sign into your LiveText account
  2. Click the "Documents" tab and then click "Inbox"
  3. Click to open the document "Spring 2008 LiveText Form Information - for Citadel faculty"
  4. Click Spring 2008 Assessment Courses, Time Line, and Rubrics to locate and open any rubrics you may need to complete your forms.
    (NOTE: Some forms do NOT require rubrics.)
     
    When you have the rubric open (or printed):
  5. Click  your name.
  6. Click one of the assessment links to open the LiveText form
  7. Refer to the appropriate rubric (if necessary), and type or click the drop-down boxes to enter the information requested and/or your comments
  8. Click "Submit form"
  9. Repeat the above for each of the candidates enrolled in your course
  10. Repeat the above for each assessment link in your area, as appropriate.

Enter an optional Name or identifier for your question (e.g. "Q1"), phrase the Question, and enter optional Pre-text and Post-text fields. When you are finished editing the question, click the Save link.
Pre-text and post-text might include instructions, explanations, or examples for a given question. For instance, you might want dates or telephone numbers to be entered in a specific format.

For each dropdown choice, enter a Name and Value (e.g. "Illinois","IL") and click the Add button. By default, the Value field is used, but you can opt not to use it by unchecking the "Use 'Value' Field" checkbox. Note that after you add a choice, you have the ability to Edit or Delete the choice and set the Order by clicking the accompanying buttons.
